We are eager to get your child in for therapy, but we are missing important documentation. You marked that your child has a formal diagnosis, but were unable to upload it.
It is necessary for us to have diagnostic documentation in order to move forward with therapy services. Once we have it, we will work to connect with you soon about the support options available for your child. Please check your email inbox for more information.
You have a few options when it comes to submitting your child’s autism diagnosis:
- Have your child’s physician fax it to us at (317) 520-8200,
- Upload directly through our website, or
- Print a copy and bring it to your local Hopebridge Center.
